Transitions Treatment Services does not condone or engage in discrimination based on age, sex, color, culture, disability, ethic group, gender identity, race, religion, sexual orientation, national origin, marital status, or socioeconomic status. Transitions Treatment Services will not discriminate against any individual regardless of payer source whether it be Medicare, Medicaid, CHIP or the inability to pay. Services offered at a discounted rate to individuals who qualify based on family size and income under Sliding Fee Schedule. Adjustments discounting the fee shall be performed at the time of the visit entry. In order for the Sliding Fee Discount, the patient must present proof of household income. Tax returns, pay stubs within the previous 3 months, Social Security or Oklahoma DHS documents are acceptable. Transitions Treatment Services will actively attempt to understand the diverse cultural backgrounds of the clients with whom they work. This includes, but not limited to, learning how counselor's own cultural/ethic/racial identity impacts her/his values and beliefs about the counseling process.
